10 Day Intensive Certificate Course in Malaria Microscopy

Early diagnosis and prompt, effective treatment is the basis for the management of malaria and key to reducing malaria mortality and morbidity. Demonstration of the presence of malaria parasites prior to treatment with antimalarial drugs is fundamental to this goal, as clinical diagnosis has poor accuracy and leads to over-diagnosis of malaria with resultant poor management of non-malarial febrile illness and wastage of antimalarial drugs. While microscopy remains the mainstay of parasite-based diagnosis in most large health clinics and hospitals, the quality of microscopy-based diagnosis is frequently inadequate for ensuring good health outcomes and optimal use of resources. An acceptable microscopy service is one that is cost-effective, provides results that are consistently accurate and timely enough to have a direct impact on treatment. This requires capacity building and a comprehensive and active quality assurance (QA) programme.

The 2014 -2015 National Malaria Strategic Plan for the country aim to achieve a malaria free Nigeria by 2020 where we would have attained the pre-elimination phase with slide positivity rate below 5%. In this plan, 80 -100% of febrile patients in both the private and public sector facilities should be tested using a quality assured diagnostics – microscopy or RDTs by 2018/2020. There are current challenges in the diagnosis of malaria especially as the epidemiology of malaria continues to change. In 2010, malaria prevalence in the community among children less than 5 years was 42%. In 2015 this figure was 27%. Most tests are likely to be negative especially in the dry seasons while parasitaemia will continue to decrease. This makes the acquisition of skills to detect low level parasitaemia and greater care being taken to declare a malaria blood film negative. We have mastered all malaria diagnosis and we are able to engage and deliver quality diagnosis in any form. This is our strength as well as our current expanded capacity to serve as a clinical trial site for malaria test evaluation. Our current capacity to deliver learning has being upgraded and so are able to deliver with great simplicity and engagement malaria diagnosis and challenges around it.

Aim: The primary aim of this certificate course in malaria microscopy is to ensure that microscopy services are manned by competent staff. This course also introduces the participants to the institution of quality assurance in their routine activities on malaria microscopy/RDTs.

Specifically, this course will build the capacity of Microscopists on the fundamentals of malaria microscopy (including RDTs); introduce the participants to Quality Assurance on malaria diagnosis and Malaria slide banking. Furthermore to build and establish the skills to differentiate the Plasmodium species, detect very low parasitaemia, quantification of malaria parasites, recognition of artifacts that are commonly referred to as parasites, microscope maintenance from the standpoint of biomedical engineering, ethics and leadership in pathology and the development of Quality control procedures in the diagnosis of malaria. The course will speak clearly to current National malaria diagnosis and treatment guidelines as it relates to diagnosis and its role in promoting effective malaria case management. Templates of standard operating procedures for malaria diagnosis will be provided to each participant. The approach to malaria negative smear reporting will be fully elucidated to ensure that negative MP test results (which are a common post-training feature) are arrived at using best standards. It is on record that our trained participants are now able to recognize malaria parasites from artifacts and pseudo-parasites which there were used to reporting as false positive smears and these are changed when they return to their facilities.

Target: Malaria Microscopists from within and outside Africa involved in malaria diagnosis (International Travel/Expatriate Clinics; Laboratory Scientists in the private sector; Research Microscopists (Drug and vaccine trials); Microscopists in National Malaria Control Programmes (NMCPs); Regional/Zonal/State/District/Local Area microscopists etc.

Format of the Course: Lectures, problem-based and hands-on activities with continuous assessment from Day 3 to Day 10; presentation by participants and real-time facility-based activity. Sunday is a free day.


Duration: This is a 10-day intensive course covering several modules including ethics in pathology and leadership. In addition it covers uniquely the NEW WHO MALARIA MICROSCOPY SOPs THAT ALSO MOTIVATES FOR SAFETY IN EMERGENCY SITUATIONS SUCH AS EBOLA. THE 2015 MALARIA MICROSCOPY MANUAL OF WHO IS SUMARIZED IN THIS SHORT COURSE BUT EXPANDED IN THE 10-DAY TRAINING PROGRAMME.



Course fee:  N350,000.00 including tuition, course materials, tea breaks, and lunch.

The ANDI Centre of Excellence for Malaria Diagnosis currently has three (3) WHO Level I Malaria Microscopists, Two Level II and one Level III malaria microscopists. This makes it the only Centre with the highest competency of malaria microscopists in Africa. Our facility also passed a recently conducted External Quality Assurance (EQA) assessment.
